Forms Below are the forms that an employee should complete for their withholding allowances.  Please note that both forms should be completed for an employee living in Georgia.  If you have an employee from another state, feel free to contact our office. W4 Withholding Form G4 Withholding Form Federal law requires all employers to verify the eligibility of employees to work within the United States.  This is done by completing an I9 form.  This form should be maintained at the employer's location.  Failure to obtain this form could result in penalties / fines beginning at $1000 per day.
